Circumstances Leading to Death (Normally from War Diary)
Samuel is listed as being killed in action on 25 Sep 1915 at Hooge. On the 25 September 1915 at Hooge the allies launched a diversionary attack to deflect attention from the main assault at Loos, 20 miles south of Ypres.The war diary for the day is transcribed below (not pleasant reading):-
In the field (at Ecole) - Sept 25th: At appointed time 3.50am opened fire with all 3 guns. During Bombardment Cpl Hather No 1 of D A Gun was killed by a shell exploding in the gun, both man and gun being blown to pieces (cause unknown). Remainder of Det suffering from severe shock.
Lt S.S. Forsyth was killed about 6 am, shot through the head. B Gun put out of action (Rifle ? struck by piece of shell). Remaining gun being buried. Cpl McCallum took charge and retired in the evening after reporting to O.C. South Lancs. Lt Hankins arrived at Ecole and took command at 11pm then proceeded to the trenches. 68 rounded received from 40th Bde a.c.

- Brother of John Cusack Forsyth and Cusack Grant Forsyth who also fell and Frederick Richard Forsyth who was seriously wounded but survived. A fourth brother, Arthur Forsyth died of malaria in Northern Nigeria in 1909 whilst serving with the African Frontier Force.
